PERSONAL RELEASE FORM FOR DISTANCE LEARNING CLASSROOM TELEVISING AND VIDEOTAPING

 

Distance learning classes at CSUSB Distance Learning classes are videotaped and temporarily stored in the Media Services video library.  The videotapes are used as back up in the case that transmission problems should occur with our remote sites.  Faculty and students on our main campus for reference and training can also view them.  The CSUSB Academic Computing and Media Department reserves all rights for the use, display and editing of these videotapes for nonprofit educational purposes.        

 

I understand that as an enrolled student in a distance learning class, my audio and video image will be transmitted to other audiences at remote sites, I will hold the Academic Computing and Media Department and the California State University San Bernardino and their employees harmless from any liability in connection with this production or transmission.
